What you'll do

• Orient new patients to program rules, policies, procedures, and rights
• conduct intakes, discharge planning, assessments, and treatment plans
• administer biopsychosocial assessments and screening tools
• monitor changes and relapse indicators
• identify and refer high-risk patients
• provide individual and group counseling as clinically necessary
• coordinate referrals and document services
• participate in team meetings, training, and community outreach.
