Skilled Home Care Nurse

What you'll do

• Conduct head-to-toe assessments and monitor vital signs
• evaluate physical condition and changes from baseline
• monitor chronic conditions and identify potential complications
• report significant findings
• document assessments and interventions
• assess home care needs and develop care plans as applicable
• complete initial and recurring assessments and supervisory visits
• perform ordered home visits and nursing tasks
• update policies and procedures
• collaborate with the interdisciplinary team
• maintain participant vaccination records.
