What you'll do

• Delivers outreach services to mentally ill individuals in the community, providing in-home mental health intervention.
• Collaborates with another team member to perform psychosocial evaluations and assess mental health service needs.
• Creates and executes short-term service plans.
• Responds promptly to dispatch calls, traveling from the office to the clients’ homes within two hours.
• Ensures seamless program operations.
• Ensures uninterrupted comprehensive care.
• Prepares case histories and case records.
• Encourages resistant clients to accept mental health services.

What we're looking for

Education

Master’s degree in social work

Experience

Minimum of one year experience as a Social Worker in a health care setting required
