Hospice Team Physician

The role helps the team coordinate care and support patients and families through end-of-life decisions.

What you'll do

• Contribute to interdisciplinary care planning, team discussions, and quality improvement
• provide medical care and guidance, including symptom management and medication review
• support patients when the attending physician is unavailable
• develop, review, and update individualized plans of care
• complete certifications, recertifications, and face-to-face encounter attestations
• collaborate with attending physicians, providers, patients, and families
• participate in on-call coverage for continuous 24/7 physician availability
• support hospice admissions, recertifications, revocations, and discharges
• maintain accurate, timely, compliant medical documentation.
