The Nurse Case Manager role is onsite in Syracuse, NY. The role combines nursing assessments and medical coordination with case management, advocacy, and support for participants’ stability in the community.

What you'll do

• Monitor the health and well-being of participants
• attend admission, intake, discharge, and annual meetings
• review medical records and medications
• coordinate medical services and appointments
• complete nursing assessments and Plans of Nursing Services as needed
• document care and follow-up
• provide CTI case management and referrals
• support medication-assistance training and staff competency
• respond to assigned on-call matters outside agency hours.
