What you'll do

• Perform resident assessments routinely and when conditions change or residents return from hospital
• observe, assess, and report changes
• coordinate timely assessments and CAAs with interdisciplinary departments
• complete MDS assessments, quarterly reviews, CAAs, and care plans
• maintain MDS schedules
• and transmit MDS data to the state with accurate documentation and logs.
