Advanced Practice Provider — Infectious Disease

What you'll do

• Provide inpatient and outpatient infectious disease follow-up
• evaluate and manage acute and chronic infectious conditions
• collaborate with physicians, pharmacists, nursing, and care management on individualized treatment plans
• assist with antimicrobial stewardship and infection prevention
• participate in patient education and transitions of care
• coordinate with referring providers
• use Epic EHR and integrated clinical tools.
