The specialist provides short-term in-home and community-based support to referred families, helping build parenting skills, connect families with resources, and promote stability and well-being.

What you'll do

• Conduct outreach and home visits
• complete intake and family assessments
• develop individualized program plans and service goals with families
• provide education and coaching on parenting, child development, stress management, and family functioning
• connect families with community resources
• advocate for families
• provide approved transportation to appointments
• communicate with referral sources
• and maintain timely, confidential documentation.
