What you'll do

• Supervise RNs, LPNs, therapists, and home health aides
• perform start-of-care and recertification visits, including OASIS assessments
• oversee admissions, plans of care, and case coordination
• review clinical documentation for accuracy and timeliness
• maintain compliance with state and CHAP standards
• lead quality improvement and staff training
• serve as clinical contact for physicians, hospitals, and case managers
• share in RN on-call coverage.
