This role supports the mission of At Peace Health Care Agency by providing essential care to individuals with intellectual and developmental disabilities. This role exists to provide person-centered care, foster independence, and ensure the safety and dignity of individuals with disabilities. The professional will implement individualized care plans, assist with daily living activities, and provide transportation to appointments to help clients achieve their personal goals.

What you'll do

• Deliver hands-on support with daily living activities, personal care, and mobility.
• Foster independence and community involvement for individuals with disabilities.
• Implement and adapt individualized care plans and behavioral strategies.
• Ensure a safe, respectful environment that protects individual rights and well-being.
• Support individuals with Autism Spectrum Disorder and other diverse needs.
• Maintain strict confidentiality in compliance with HIPAA standards.
• Provide transportation and accompany individuals to medical, therapy, and community appointments.
