Registered Dietitian — Fairlane Dialysis

What you'll do

• Conduct initial and follow-up nutrition assessments with patients and support systems
• evaluate nutritional status and develop individualized diet plans
• educate patients, caregivers, and unit staff
• coordinate with hospitals, extended care facilities, home health agencies, senior centers, and community partners
• monitor adherence to nutrition plans
• recommend nutrition-related medication or supplement adjustments
• review monthly lab results
• recommend feeding and supplemental options
• respond to urgent nutrition concerns
• consider medication-nutrition interactions.
