Mental Health Counselor/Assessor

Description of Position :. The Mental Health Assessor/Counselor provides initial and annual assessments, including DSM diagnoses and collaborative treatment plans, for individuals with Severe and Persistent Mental Illness. This role also includes counseling, care transitions, and crisis intervention services. Services may be provided in-home or in community settings when medically appropriate.
