Case Manager

What you'll do

• Assess resident needs and develop, implement, and monitor individualized plans
• coordinate healthcare, rehabilitation, and supportive services
• advocate for residents
• coordinate insurance authorizations and utilization management
• submit clinical documentation and monitor coverage and authorization dates
• plan discharges and transitions
• coordinate post-discharge services and resources
• maintain case management records and regulatory compliance
• communicate with residents, families, clinicians, insurers, and external providers.
