What you'll do

• Conduct pre-admission clinical assessments
• advise on patient placement, rehabilitation options, funding, and level of care
• coordinate communication among referral sources, patients, families, payers, admissions, operations, and clinical teams
• identify and address admission barriers
• develop referral relationships and territory and account plans
• deliver clinical education and presentations
• monitor market and referral trends
• document customer interactions and referral activity in the CRM
• collaborate across admissions, operations, clinical leadership, finance, contracting, marketing, and executive leadership.
