What you'll do

• Provide subject matter expertise and operational support to housing services staff
• coordinate applications, outreach, home visits, referrals, crisis intervention, case management, and advocacy
• lead staff meetings and consult on participant housing plans
• assess participant needs and develop, implement, and monitor service plans
• coordinate with healthcare providers, social service agencies, shelters, and other providers
• support staff at clinic locations
• develop training aids and onboarding support
• monitor productivity
• manage housing referrals requiring follow-up
• assist with monthly reports
• maintain knowledge of community resources
• uphold confidentiality and HIPAA policies.
