Social Worker — Adult Day Health Care

The Social Worker role is onsite in Modesto, CA. Compensation is $28–$38 per hour. The position supports the Adult Day Care Center team in evaluating patients and developing individual treatment plans with their physicians. The social worker contributes to patient care by assessing psychosocial needs, participating in care plans, planning interventions, and reassessing progress. Accurate records and quarterly reports help the team review and continue appropriate plans of care.

Why this role

Accepts specified master’s degree pathways with at least one year of relevant experience or a bachelor’s degree in social work with two years of relevant experience; current CPR certification and a valid driver’s license are required. Bilingual ability is a plus.
