What you'll do

• Assess home health clients, including OASIS assessments
• reevaluate nursing needs and outcomes
• develop and revise plans of care
• coordinate with clients, families, physicians, and care teams
• follow infection-control practices
• document care
• supervise and teach LVNs/LPNs and CNAs
• provide on-call services as assigned.
