Registered Nurse (RN) Case Manager

What you'll do

• Conduct in-home assessments and develop individualized care plans
• coordinate nursing and home care services
• collaborate with the multidisciplinary team at the day center clinic
• participate in care-planning meetings and maintain documentation
• travel locally to visit patients
• maintain an active Pennsylvania RN license and valid driver’s license.
