Job overview

The Registered Nurse (RN), Home Health Admissions role is onsite in Kansas City, MO, and surrounding areas, with a Monday–Friday day schedule and pay of $75,000–$90,000 per year. The nurse is the first point of contact between patients and the hospital and supports Phoenix Home Care and Hospice’s home-care mission. The RN assesses patients, interprets diagnostic information, and develops individualized plans of care based on physician orders. Through skilled visits, education, wound care, medication management, and coordination with other clinicians, the role contributes to patient care and continuity during home health admissions.

What you'll do

• Conduct skilled visits and day-to-day patient interactions
• assess patients and contribute to individualized plans of care based on physician orders
• educate patients about health conditions including CHF, COPD, and diabetes
• provide wound care and medication management
• change dressings and bandages as prescribed
• document patient condition and report significant changes to physicians
• collaborate with nurses, physical therapists, speech therapists, and occupational therapists.
