Hospice Medical Social Worker

What you'll do

• Assess psychosocial status of patients and families and communicate findings to the interdisciplinary group
• conduct social evaluations and provide counseling or crisis intervention as needed
• educate patients and families about advance directives
• contribute to individualized care plans and attend team meetings
• coordinate referrals and community resources
• maintain clinical notes
• participate in on-call duties.

Why this role
