Care Manager – Family Reentry Support and Help (FRESH)

The Care Manager – Family Reentry Support and Help (FRESH) role is hybrid in Wadesboro and Polkton, NC, including work at Anson Women’s Correctional Facility and from home. Compensation is $42,000–$45,000 per year, based on experience. Coastal Horizons Center’s FRESH program supports family reentry and the child-parent relationship for incarcerated parents and their children. The care manager provides case management and related services in correctional and community settings. The role supports transition and reentry by coordinating services focused on family relationships and helping parents and children navigate the process.

What you'll do

• Provide case management and related services focused on the child-parent relationship and reentry or transition process for incarcerated parents and their children in correctional and community settings
• complete required NCDAC and Parenting Inside Out training
• maintain computer-based records and communications
• work a flexible schedule, including travel.

Why this role

40 hours per week, Monday–Friday. Associate degree with four years’ experience or bachelor’s degree with two years’ experience required; one year working with individuals, families, and children in correctional and community settings required. North Carolina Peer Support Specialist certification or NCASPPB registration is preferred.
