What you'll do

• Coordinate patient recruitment, enrollment, intake, referrals, follow-up, and retention
• manage patient panels and conduct assessments in person and by phone
• provide education on HIV prevention and treatment and respiratory care
• conduct phone, clinic, and field outreach, including weekend and evening events
• support screenings and harm-reduction education
• maintain accurate encounter and referral records
• follow HIPAA requirements
• and assist with grant deliverables and program workflow reviews.
