Community Health Worker — Functional Family Therapy–Gang Program

What you'll do

• Complete contact notes and transfer documentation to the EHR
• upload assessments and data
• attend program-related community events
• practice skills with clients’ families between sessions
• collaborate with clinicians to support youth and family engagement
• attend intakes and sessions as needed
• and mentor participants in skills and relationship building. Statewide travel is required.
