What you'll do

• Provide in-person enrollment assistance and information on Minnesota health insurance programs
• conduct basic income screening
• help clients complete applications, advocate on their behalf, and follow up with agencies
• assess patient needs
• arrange referrals and appointments with clinic departments
• assist with charity-care applications
• maintain program data and statistics
• provide community outreach and education.
