What you'll do

• Assess WIC participants’ health histories, dietary intake, anthropometric measurements, and blood work to determine nutritional or medical risks
• provide individualized nutrition counseling and appropriate food packages
• document risk codes, food packages, and care plans
• determine and provide nutrition education contacts
• implement the High Risk Plan and contribute to the Biennial Nutrition Education Plan
• counsel prenatal participants on infant-feeding choices
• refer participants to other services when appropriate
• maintain education materials and required documentation
• coordinate programs with the Director
• provide outreach as requested
• and travel to other county WIC clinic sites as requested.
