What you'll do

• Complete social-services components of resident assessments and contribute to individualized care plans
• encourage resident and representative participation
• support preadmission evaluations and PASARR Level 1
• coordinate discharge planning and community services
• connect residents with financial, legal, and community resources
• document social-service actions
• support advance care planning and resident rights
• assist with grievances and report suspected abuse, neglect, or exploitation
• coordinate with facility departments and participate in care-plan and quality meetings.
