Substance Abuse Case Manager

The Substance Abuse Case Manager role is onsite in Washington, DC, with compensation of $30–$40 per hour. The position supports outpatient substance abuse treatment services at Creative Pathways, Inc. The role helps clients address alcohol or other drug addiction by providing counseling, individualized treatment planning, referrals, education, and follow-up care. It also supports continuity of care through client assessments, case documentation, and aftercare planning.

What you'll do

• Provide substance abuse counseling and treatment in an outpatient setting
• complete individualized treatment plans and provide referrals and education
• assess clients and family systems and identify areas needing support
• maintain case records and required documentation
• provide follow-up and aftercare planning and discharge inactive clients
• provide emergency services during work hours and after hours as assigned
• attend staff meetings and supervisory conferences.
