What you'll do

• Lead and supervise social work, participant advocacy, and behavioral health teams
• coach and manage direct reports
• oversee participant advocacy and enrollment
• address complex participant concerns
• partner with the interdisciplinary team on assessments and care planning
• coordinate staffing and assignments
• build community and agency relationships
• review disenrollment risks and coordinate interventions
• support continuous improvement and participant-centered service delivery.
