MDS/Care Plan Coordinator

The MDS/Care Plan Coordinator role is onsite in Tallahassee, Florida. Compensation is not specified. What you'll do

• Coordinate resident assessments and care plan completion in accordance with applicable rules and guidelines
• track Medicare and Medicaid case-mix documents
• conduct concurrent MDS reviews
• integrate information from nursing, dietary, social services, restorative, rehabilitation, and physician services
• maintain resident privacy and rights
• complete MDS corrections
• report on assessment and care plan functions
• consult department supervisors
• evaluate resident conditions and medical data for assessment needs.
