Care Transition Coordinator – Hospice

What you'll do

• Serve as liaison among referral sources, patients, caregivers, and clinical teams
• conduct structured sales calls

• track sales activity in CRM tools and work toward referral and admission goals
• collaborate with clinical and operational teams
• facilitate transitions into home health or hospice
• document in CRM and EMR systems.
