What you'll do

• Support individuals with budgeting and money management, hygiene and household tasks, social activities, and community access
• maintain health and safety
• complete daily progress notes and quarterly reports
• follow individualized service plans
• attend training and biweekly supervision
• maintain confidentiality
• facilitate conflict resolution
• attend appointments and assessments
• coordinate with families and healthcare professionals.
