What you'll do

• Develop, implement, and direct social services functions
• maintain quality assurance and corrective action plans
• coordinate with other departments
• ensure forms, reports, and progress notes are complete
• participate in community planning, discharge assessments, and discharge plans
• coordinate programs with residents, families, staff, and community agencies
• inform families about support agencies and financial assistance
• protect resident confidentiality and rights
• monitor whether residents’ social and emotional needs are met.
