What you'll do

• Oversee day-to-day Social Services Department operations
• identify and address residents’ psychosocial needs
• participate in care plan meetings and interdisciplinary conferences
• complete assessments and documentation
• assist with admissions, transfers, room changes, and discharges
• coordinate discharge plans and follow-up
• communicate with residents and families
• advocate for resident rights and preferences
• provide emotional support and informal counseling within the role’s scope
• coordinate referrals and community resources
• address grievances
• maintain confidentiality and comply with HIPAA
• participate in facility meetings and quality improvement activities.
