Clinical Case Manager - Co-Occurring

Gateway Rehab Center (GRC) is seeking a Clinical Case Manager to support the day-to-day operations of the Co-Occurring program. This role is responsible for conducting assessments, assisting patients with appointment scheduling, and collaborating with the treatment team to develop and implement continuing care plans. The position also coordinates continuing care services, provides exceptional supp
