The Mental Health Professional, Targeted Re-Entry Services role is onsite in Mount Vernon, WA. Pay is $29.94–$46.28 per hour. The position supports Lifeline Connections' jail services and community-based behavioral health care coordination for participants returning to the community. The professional assesses participant needs, develops individualized re-entry care plans, coordinates services across correctional, healthcare, and community systems, and supports recovery and community integration. The work contributes to Lifeline Connections' mission of supporting people affected by substance use and mental health conditions.

What you'll do

• Conduct biopsychosocial, behavioral health, and health-related social needs assessments
• identify strengths, barriers, risks, and service needs
• develop and monitor individualized re-entry care plans
• provide targeted case management and behavioral health support
• coordinate transitions among correctional facilities, providers, housing, employment, and community organizations
• facilitate access to treatment, medical care, benefits, transportation, housing, and social supports
• identify crises and provide intervention, de-escalation, or referral consistent with protocols
• collaborate with multidisciplinary teams and community partners
• maintain compliant clinical documentation and meet productivity, billing, and reporting requirements.
