What you'll do

• Own and grow referral relationships with hospitals, skilled nursing facilities, rehabilitation centers, senior living communities, and other community partners
• prospect, make cold calls, follow up, and develop tailored solutions
• align Home Instead services with partner needs and care gaps
• identify referral opportunities and reduce referral friction
• track activity and outcomes in CRM
• conduct field outreach and engage multiple stakeholders
• maintain long-term partner relationships and reactivate stalled relationships
• coordinate with intake and care teams to reduce referral drop-off.
