Youth Diversion Case Manager

What you'll do

• Coordinate screening intake meetings
• work with the program coordinator and community partners to determine 12-week diversion contract requirements
• meet weekly one-to-one with enrolled youth
• provide support toward program completion
• communicate with youth and families
• liaise with collateral agencies
• identify community resources
• advocate for families
• attend weekly team and supervision meetings
• perform additional duties assigned by the coordinator.
