What you'll do

• Provide housing-focused case management
• coordinate housing support services, referrals, and eligibility
• develop and monitor housing and care plans
• oversee referral, placement, move-in, and retention processes
• maintain compliant client records
• address barriers and crises
• coordinate property and tenancy matters
• and supervise, train, and coach assigned staff.
