Registered Nurse (RN) Case Manager — Coordinated Care

The Registered Nurse (RN) Case Manager — Coordinated Care role is onsite in Jackson, MS. Compensation is not specified. The role supports coordinated care and discharge planning using the UMC nursing and case management model. The RN coordinates patient-centered discharge plans, assesses clinical needs, arranges services and works with care teams, patients, families, payers and community resources. The purpose is to support safe, timely transitions, continuity of care and effective use of health resources while addressing barriers to discharge and readmission risks.

What you'll do

• Coordinate multidisciplinary care plans and discharge needs
• assess clinical discharge needs using nursing processes and evidence-based practice
• conduct follow-up assessments and screenings
• collaborate with providers and care teams
• document in medical records
• identify and resolve discharge delays
• arrange post-acute services and resources
• support patient flow and safety
• and maintain an active RN license. Requires an active RN license and at least one year of acute-care experience
• case-management or utilization-management experience is preferred.
