PHC Coordinator - San Antonio

What you'll do

• Manage daily caregiver schedules and coverage
• match caregivers with clients based on skills, personality, and availability
• serve as the primary contact for clients and families
• address concerns, condition changes, and service requests
• collaborate with nursing and clinical staff
• support caregiver onboarding and scheduling
• maintain documentation, service logs, and billing timesheets
• monitor EVV, attendance, and service quality
• maintain compliance with Texas HHS and Medicaid PHC regulations
• respond to call-offs, emergencies, and staffing gaps.
