What you'll do

• Assist with activities of daily living, comfort measures, mobility, repositioning, and transfers
• observe and report changes to nursing staff
• maintain clean, stocked, and safe patient rooms
• support families and visitors
• follow individualized care plans and hospice policies
• collaborate with nurses, social workers, chaplains, volunteers, and other team members.
