What you'll do

• Evaluate, diagnose, treat, and manage acute and chronic conditions
• take patient histories and conduct physical examinations and clinical assessments
• develop individualized care plans and coordinate specialty referrals
• document encounters and treatment plans in the VA electronic health record
• collaborate with the CBOC care team
• participate in credentialing, privileging, and quality assurance
• support preventive care and chronic disease management.
