Memory Care Resident Care Assistant / Personal Care Assistant

The Memory Care Resident Care Assistant / Personal Care Assistant role is onsite in Raynham, MA, with pay of $19 per hour. The position is per diem with all shifts available and supports the community’s Wellness team in serving older adults. The assistant provides personal care, supervision, and companionship to help residents maintain independence, dignity, comfort, and quality of life. The role follows individualized service plans, observes and reports changes, documents care, and supports residents’ daily activities and safety.

What you'll do

• Assist with bathing, dressing, grooming, toileting, hygiene, mobility, transfers, and meals
• provide companionship and emotional support
• encourage participation in community activities
• respond to resident requests and emergency-call systems
• observe and report changes to a nurse or supervisor
• follow service plans and safety procedures
• provide permitted medication-management assistance after required training
• document care
• maintain resident areas and protect privacy and dignity.
