What you'll do

• Assist with transitional care for hospitalized clients
• coordinate discharge plans and treatment to help decrease subsequent hospitalizations
• work with a multidisciplinary team and outside agencies
• transport clients and conduct home visits
• meet service levels and health home targets
• maintain required licensure and credentialing documentation.
