What you'll do

• Support patients through outreach calls, telehealth, and face-to-face visits

• encourage follow-up with healthcare providers
• support medication adherence through delivery and pharmacy coordination options
• conduct screenings and assessments in the EHR
• educate the behavioral health team about rehabilitation services
• connect patients with community resources
• coordinate with providers
• travel locally for sessions
• retrieve clinical and non-clinical data
• participate in related projects and programming.
