What you'll do

• Build and maintain relationships with medical professionals, hospitals, and healthcare organizations
• represent the facility’s mission and care standards
• participate in meetings, calls, and in-services
• maintain referral-tracking logs
• prepare end-of-day reports
• and collaborate with interdisciplinary teams to support transitions of care and communicate resident needs.

Why this role

A valid driver’s license is required because travel between facilities and partner organizations is essential. Current RN, LPN, or LSW licensure is preferred; an associate’s degree in a related field is required and a bachelor’s degree is preferred.
