What you'll do

• Conduct skilled nursing visits and start-of-care, recertification, resumption-of-care, and discharge assessments
• complete OASIS documentation and care plans
• reconcile medications and educate patients
• provide wound care, disease management, vital-sign monitoring, and IV infusions
• coordinate with physicians, families, therapists, and the care team
• document patient progress and changes in condition.
