What you'll do

• Conduct individual, group, and family psychoeducational and process groups
• contribute to treatment plans and attend planning meetings
• assess patient and environmental safety and report concerns
• identify risk factors and interventions for special precautions
• complete assessments and documentation
• coordinate discharge planning with the multidisciplinary team
• conduct post-discharge follow-up as indicated
• maintain community resource information
• facilitate community education and support groups as appropriate
• and contribute to quality improvement.
