Hospice Social Worker

The Hospice Social Worker role is onsite and on the road in Hialeah, FL. Compensation is $26–$35 per hour. The social worker supports Moments Hospice’s Social Service team in providing care to terminally ill patients and their families. The role assesses psychosocial needs, provides counseling and case management, and connects patients and families with community resources and bereavement support. Coordinating services and implementing care-plan provisions helps address patient and family needs throughout terminal illness and at the time of death when requested.

What you'll do

• Provide psychosocial services to terminally ill patients and families
• contact the patient, family, and caregiver within 48 hours of admission
• complete psychosocial assessments for the patient chart within 24 hours of completion
• assess patient and family psychosocial and anticipatory grief needs
• implement care-plan provisions, including social-service goals, supportive counseling, problem solving, community referrals, and pre-bereavement and bereavement care
• participate in weekly interdisciplinary team meetings
• assist with funeral arrangements if requested
• provide counseling during terminal illness and at death if requested.

Why this role

Current social worker license and valid driver’s license and auto insurance required. Requires a master’s degree in social work or a bachelor’s degree in social work with at least one year of healthcare experience, plus at least two years of social work experience in healthcare or a social services agency. Hospice experience is a plus.
