What you'll do

• Conduct direct-care rounds and assist with activities of daily living
• maintain positive relationships with residents and patients
• notify the charge nurse of changes in condition
• obtain vital signs, weights, and heights as instructed
• complete assigned duties and documentation
• coordinate new admissions, transfers, and discharges in the Transitional Care Unit
• maintain a safe, clean work area and follow infection-prevention practices.
